Family Help

Family Help is here for you when you need support with the challenges your family are facing. This could include:

  • Helping you manage multiple or complex issues
  • Working with schools, health services and other professionals with you
  • Supporting routines, relationships and wellbeing at home
  • Helping you build confidence and long-term stability for your family

Family Help is flexible and centred around you. We will work at your pace and focus on what will make the biggest difference. Our aim is to help families feel stronger, more confident, and better connected to support in their community.

We use a family-led approach, helping you identify relatives, friends and other people in your community who may be able to offer support. By bringing together your wider support network, we can help create positive, lasting change for your family.

Your Family Help Lead Practitioner

When you are working with Family Help, you will have a Family Help Lead Practitioner. They will be your main point of contact throughout your journey.

They will:

  • Get to know your family and understand your strengths, challenges and goals
  • Listen to everyone in your family, including children and young people
  • Help you create and review your Family Help Assessment and Plan.
  • Arrange family meetings and support family-led decision making.
  • Work with other professionals and services so you are getting the support you need. Unless someone is at risk, we will always ask for your consent before we share your information with another team. 

Family Help Assessment and Plan

When there is a lot going on for your family, it can be difficult to know where to start. A Family Help Assessment and Plan helps you think about the things that are going well and the things you would like to change. With your Family Help Lead Practitioner, you can plan how those changes might happen. Your plan will change over time as some things become easier for your family, but other things might become more difficult. Your Family Help Lead Practitioner is there to support and guide you through this journey. 

A Family Help Assessment and Plan is useful when your family is facing several challenges, or when things feel difficult to manage on your own. It helps your family and the professionals supporting you agree on what is important, set shared goals, and work together towards positive change.
