Citizen Portal for assessment and reviews

The Citizen Portal is a secure website. It is used during an Education Health and Care (EHC) assessment and review. Parents/carers and young people as well as schools and professionals can use it to:

  • apply for an EHC assessment
  • securely share sensitive information about a child or young person
  • track the progress of an EHC assessment and review
  • view documents including:
    • letters and advice from professionals
    • the Education Health and Care Plan (EHCP)
    • EHC assessment and review documents 

The Citizen Portal makes it easy to share and view information.

How to set up your Citizen Portal 

To use the Citizen Portal for EHC assessments and reviews, you will need to do the following:

Register for the Citizen Portal

You might already have an account. Parents and carers use it when applying for a school place. 

If you don't have one, that's okay. Just follow the steps below.

  1. Go to our Citizen Portal website
  2. Click on 'register an account'
  3. Read the 'Data protection' information
  4. Tick 'I agree to my personal details being used as above' if you are happy with how we will use your data
  5. Click 'Continue'
  6. Fill in your details
  7. click 'Submit'

Set up 2 step verification

You need to set up 2 step verification to access the EHCP section on the Citizen Portal. It is used to keep your sensitive data more secure. 

When 2 step verification is set up, a verification code will be sent to your email each time you sign in. You will need to enter this code before you can access your account. 

Follow these steps to set up 2 step verification:

  1. Log into your Citizen Portal Account
  2. Click on 'Special Educational Needs and Disabilities'
  3. You will be told that 'This area requires two step verification'. Click 'Enable Two Step Verification now'
  4. Change 'Preferred method' to 'Email'
  5. Click 'Save'
  6. Log in again
  7. Enter the verification that is sent to your email

Add your child (if you are a parent or carer)

When you access the EHCP section of the Citizen Portal, you might already see the name of your child. This happens if an assessment for your child has already begun.

If you do not see their name, then you can add them by following these steps:

  1. Click '+ Add child'
  2. Enter their details
  3. Click 'Add child' at the bottom of the page 

You will then be able to view your child's profile and request an assessment.

Forms for Parents/Carers and Children/Young people

You will need to submit information during an EHC assessment. We collect this information through forms. You can do this by either:

  • filling and submitting forms found on the Citizen Portal
  • downloading and completing the forms linked below, and then uploading and submitting them in the Citizen Portal

Parent Carer contribution and request for a statutory Education, Health and Care Needs Assessment

This form is used to:

  • request an EHC assessment for a child or young person
  • collect information from parents and carers about their child's needs

The form will need to be completed for a referral from a professional, as well as a self-referral.

Child or Young Persons Views 

 

This form lets a child or young person share their views about:

  1. Important things about me are...
  2. If I could, I would...
  3. Things that help me be more independent...
  4. The things that help me the best are...
  5. How I can be included in the process...
  6. A great life would look like...

This form should be completed with the child or young person.

You can choose to use either the plain, bubble or symbols template.

Parent/Carer Views – Question List This form gives the parent/carer the opportunity to add information to the assessment with a series of 8 questions.  N/A

Parent or Carers reply to Agree or Disagree and suggest changes to Draft EHC Plan

This form is used in response to the draft plan. It will ask:

  1. for some personal information
  2. whether you agree or disagree with the plan
  3. about your placement preference
  4. if further discussions are needed

Upload or Change an existing Photo of the Child or Young Person we currently hold. This form allows a photo to be uploaded or changed. N/A
